CARE COORDINATION
It takes a village to raise a child! We can help families build that village through care coordination, which includes: brief assessment, referrals/resources, and support accessing services.
Care coordination is a service aimed at helping families access the support that best fits their needs. We provide 2-4 sessions of free care coordination to caregivers of infants/children under age 6, pregnant individuals, and their partners.
Providers meet with caregivers to build trust, assess functioning, identify needs, refer to services, and support access to services and resources.
Care coordination is a free service, either billed through Medicaid or offered through grant funding. Refer/enroll by clicking below.
